Airway-adjacent shock physiology.

Immediate actions

  • Check pulse/rhythm/MAP and EtCO₂ trend.
  • Reduce excessive intrathoracic pressure if appropriate.
  • Treat tension/auto-PEEP/RV failure/hemorrhage/sepsis as indicated.
  • Review induction and sedation medications.

Likely buckets

  • Preload loss from positive pressure.
  • Medication vasodilation or myocardial depression.
  • Tension pneumothorax or auto-PEEP.
  • RV failure/PE/pulmonary hypertension.
  • Underlying shock progression.
